The Transplant Center

We attribute much of our success to a multidisciplinary approach to patient care. From the time of the first referral contact through the first year after transplantation, patients follow pathways designed to carry them through the healthcare continuum as smoothly as possible. Our "on-site" team consists of thoracic surgeons, pulmonologists, nurse coordinators, dietitians, rehabilitation specialists, social workers, respiratory therapists, financial representatives, chaplains, pharmacists, and home-healthcare specialists. As our patients recover from transplant surgery, we strive to maintain regular contact with the referring and primary-care physicians who will be sharing the care of these complex patients with our Lung Transplant Program staff.
