Print and have the Organ Donor Card proper witnessed.  Carry it with you in your purse or wallet.  It is a good idea to have one of the witnesses a member of your family.  In most states, family consent is required.  Be sure that you have discussed organ and tissue donation with your family and make them aware of your desires.
